Nondeductible IRAs and Coverdell ESAs: This form is used to report any of the following, 1) Nondeductible contributions made to traditional IRAs 2) distributions from traditional, SEP or Simple IRAs, if nondeductible contribution to traditional IRAs has ever been made 3) distribution from ROTH IRAs 4) distribution from Coverdell education savings account (ESA) and 4) conversion from traditional, SEP or Simple IRAs to ROTH IRAs.
